O: Wing and body wrap in place. Manually removed parasites from under tongue, examined under microscope - appear likely to be flukes. Drew blood from the L ulnar vein, blood pressure very poor. Small amount of unabsorbed fluids on the R hock, most of fluids from intake appear to have absorbed. Slightly more visually reactive on the R side than the L but appears visual in both eyes, fundic looks unremarkable in both eyes. Food: HF 30g LRS SQ: 42.35 cc (LRS) SQ Terbinafine: 0.37 cc (50 mg/ml compounded ) PO Meloxicam 0.5 mg-kg: 0.28 cc (1.5 mg/ml compounded) PO Calcium Slurry: 1.27 cc (100 mg/ml) PO
A: TS much lower than expected, may need to switch to more emaciation protocol for bird based on how bird processes HF and absorbs fluids. Coracoid fracture looks like it already has callus formation, may be able to just keep bird on cage rest and focus on feeding, reassess fracture stability at next wrap change.
P: Start on TID emeraid. Monitor parasites in mouth. Rx prazi for suspected flukes. Change wing and body wrap in 2d. Monitor processing of food, absorption of fluids. Place IO catheter and start on hetastarch tom if not absorbing fluids. HF BID if processing whole food. Switch from meloxicam to gabapentin for pain, older fracture likely does not have a lot of inflammation and bird is very thin.

Placed in regular cage in ICU. ------------- PHYSICAL EXAM FINDINGS ---------------- Mentation: QAR Temp (F): 104.0 Dehydration (%): 5 Heart rate: WNL Resp rate: WNL - clear on auscultation Neurologic: WNL Head/mouth/beak: Many gapeworms in mouth - glottis, choana, extending to trachea. Some fresh blood in end of glottis. Ears: NSF. Eyes: NSF. Wings: R wing lax Legs: NSF Feet: Small abrasions on plantar aspect of both feet. Cold to touch. Body: Thin. Feather condition: Ok. Tail wet. Organic debris (thorns, burs) throughout. Ectoparasites: FFs, gapeworm/fluke/maggot in mouth Radiographic findings: Right coracoid, clavicle, and scapula fractured. Some callous formation already. Initial treatment: Applied figure 8 wrap with body band. 0.28cc meloxicam, 0.37cc terbinafine, 1/2 tab oral Capstar, 42cc LRS SQ in L inguinal. Sprayed with Adams.
A: Mature HY RTHA with heavily fractured R shoulder, likely sustained from some form of blunt force trauma. Radiopaque soft callous formation seen around fracture - likely a few days old. Unsure exactly which ectoparasite is present within mouth - has a fairly heavy load, concerned for potential proliferation down into respiratory tract. May have been picked up from bird scavenging rotting food from ground. Guarded prognosis as fracture is a bit older and may not heal in alignment that allows bird to regain flight.
P: 10/28 - vet check, blood, fecal. Continue manual removal of oral ectoparasites - examine one under microscope tomorrow. rx meloxicam, terbinafine, calcium slurry. Change figure 8 wrap and body band with gentle extension of wing E3D. SF in AM, HF in PM if DNE.
